Case Management

HCN staff provide resources and referrals to help children, family, and community members meet critical needs including housing, employment, education, legal support, access to food, our on-site diaper bank, and more. Our dedicated case managers enhance families’ sense of connection to the communities in which they live and ensure that the changing needs of the child and family are met.

Housing Support

HCN supports families and individuals experiencing housing instability, including homelessness, risk of displacement, access barriers to opportunities, a legacy of exclusion, community violence, and/or systemic trauma. HCN’s staff use a holistic, trauma-informed approach focusing on emotional safety and empowerment. Core services include housing-focused case management and building life skills and economic independence pathways. Community members complete a needs assessment and are supported to develop an Individual/Family Service Plan with goals focused on housing stability, life skills, job readiness, education, financial planning, and more. Through weekly meetings, HCN staff support you to achieve your goals, maintain progress, and navigate care systems and connect to services.

Support for Gender-Based Violence Cases

In 2025, HCN launched a Gender-Based Violence Prevention, Intervention, Advocacy, and Training Program. We provide care coordination, case management, workshops, advocacy, and referrals to survivors of domestic violence and other forms of violence and their families.

Back to School & Holiday Support

To help meet the comprehensive needs of children and their families trhoughout the year, community members receiving services through HCN or one of its collaborative members, are eligible to receive clothing from HCN's clothing closet, back to school supplies, diapers and wipes, developmentally-appropriate toys and books, and other basic necessities from us as they are donated to HCN throughout the year.
