HCN offers an array of substance use education, prevention, and treatment programs that support marginalized and underserved communities and organizations in San Francisco, including Black/African American individuals, youth, and families, service providers, educators, and community members.

JABALI PREVENTION & EDUCATION

The Jabali Substance Use Prevention & Education Program supports marginalized and underserved communities and organizations in San Francisco, including Black/African American individuals, youth, and families, service providers, educators, and community member in comprehensive prevention and education efforts through campaigns, educational curriculum, and training.

JABALI YOUTH ADVOCACY

This youth-led committee empowers young voices to be heard and amplified regarding substance abuse and prevention. The program supports youth leaders aged 14-17, including those at risk and system-involved, in building advocacy skills, creating messaging platforms, and taking action to address substance use in San Francisco.

PEER-BASED (CHOPP)

The Peer-Based Community Holistic Overdose Prevention Program (CHOPP) Includes a community needs assessment, specific SUD topic training, and support from peer-based counselors, a program manager, and a case manager. It serves Black/African American residents in San Francisco at risk of opioid and polysubstance use disorders or fatal overdoses, including older adults, parents, transitional age youth, homeless individuals, LGBTQIA+ individuals, and those with carceral system involvement.

KUPONA EARLY INTERVENTION

The program provides early intervention clinical services focusing on psychoeducation, assessment, care coordination, treatment planning, and counseling for individuals. It serves youth ages 0-20 with Medi-Cal in San Francisco who are at risk of developing a Substance Use Disorder.

KUPONA YOUTH OUTPATIENT

This program provides outpatient treatment services, including assessment, therapy (individual, family, and group), and medication assisted treatment as needed for youth ages 0-20 with Medi-Cal, in San Francisco, who are actively using substances, meet criteria for SUD diagnosis, but who do not present with acute intoxication and/or withdrawal potential.

The program provides outpatient treatment services, including assessment, therapy (individual, family, and group), and medication assisted treatment for youth ages 0-20 with Medi-Cal, in San Francisco, who are actively using substances and meet criteria for an SUD diagnosis, presenting with acute intoxication or withdrawal potential.
